Job Description:

Family Practice/Primary Care opening in Freeport, New York. A health services organization on Long Island is looking to add a new, licensed Family Nurse Practitioner (FNP) and/or Physician Assistant (PA) to join their growing staff in Nassau County. Responsibilities: The Primary Care FNP/PA will:

Treat and diagnose medical conditions
Revise and prescribe medication regimen
Provide patient education
Perform other duties, as needed Qualifications:
- 2+ years of experience in a Family and/or Primary Care role/setting
- Bachelor's and/or Master's Degree
- Active NYS Family Nurse Practitioner (FNP) and/or Physician Assistant (PA) license
- Bilingual (English and Spanish)
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized Desired Skills:
  - Bilingual (English and Spanish)
