Job Description:
The Registered Nurse Case Manager plans, organizes, and directs
hospice care, emphasizing community health education and resource
utilization.
Conducts comprehensive patient and family assessments, develops and
updates individualized care plans, and administers nursing care and
medications.
Provides health education, emotional support, and self-care
training to patients and families.
Communicates effectively with physicians, caregivers, and
interdisciplinary teams, maintaining accurate documentation.
Supervises ancillary staff, participates in quality improvement,
and manages on-call duties.
Requires nursing licensure, recent acute and home care experience,
and a valid driver--------s license. Skills include strong
observation, communication, and physical ability to support patient
needs.
Salary ranges from $73,000-$99,000, with benefits like health
plans, tuition reimbursement, 401(k), and paid time off.
