Key Responsibilities: Manage patients with chronic diseases Perform routine wellness exams Diagnose and treat acute illnesses Preferred Qualifications: Experience with Epic EMR primary care experience preferred Position Details: Type: Per-Diem-possibly part time Hours Available: Various daytime and evening hours available Weekend Shifts (Optional): Saturday and Sunday, 8 am - 4 pm Support Team: A dedicated and certified medical assistant Full front desk support Patient Care Coordinator Dedicated Care-Teams to facilitate continuity and collaboration We are looking for a patient-focused advanced practitioner who is compassionate and capable of keeping up with our growing practice.
